Looking ahead to the 2024-25 school year, we'd like to provide you with the important health requirements for your students, due by August 1. Please note: per the Nevada Department of Education, up-to-date immunizations are still required from all students.

- K-8 Religious Exemption Form

K-8 Medical Exemption Form
*Medical Exemption Form must be signed by a licensed physician.
*If your child does not have an up-to-date immunization record or signed exemption form turned in by Thursday, August 1, your child will not be able to attend school until we have it on file, no exceptions. If no changes are required and your student's current immunization record or exemption form is already on file--no further information is needed.

24-25 K-8 Medication Form
All students must have a completed 2024-2025 medication form on file with the School.
- Both a physician and parent/guardian signature are REQUIRED: If you give the School permission to administer medication. This includes non-prescription, prescription, and school-stocked medications such as Ibuprofen, Neosporin, Benadryl, Tums, eye drops, etc.
- A parent/guardian signature ONLY: If you do not give the School permission to administer medication.
- Both a physician and parent/guardian signature are REQUIRED: If you give the School permission to administer medication. This includes non-prescription, prescription, and school-stocked medications such as Ibuprofen, Neosporin, Benadryl, Tums, eye drops, etc.

Any student with asthma must have a signed Action Plan on file.

The Alexander Dawson School
The Alexander Dawson School at Rainbow Mountain, an independent school located on 33-acres in the community of Summerlin, is Nevada’s first Stanford University Challenge Success partner school for students in early childhood through grade eight.
