Welcome to the Nurse's Corner for Early Childhood

Looking ahead to the 2024-25 school year, we'd like to provide you with the important health requirements for your students, due by August 1. Please note: per the Nevada Department of Education, up-to-date immunizations are still required from all students.

*If your child does not have an up-to-date immunization record or signed exemption form turned in by Tuesday, August 1, your child will not be able to attend school until we have it on file, no exceptions.  If no changes are required and your student's current immunization record or exemption form is already on file--no further information is needed.

    New EC students enrolling for the 2023-2024 school year and joining our Summer Program must submit this form to Early Childhood Support Specialist Madeleine Nazarian at mnazarian@adsrm.org.

The Alexander Dawson School
